広範囲の毛様体皺襞部裂孔網膜剥離に対する2段階手術

Two-step surgery for retinal detachment with extensive breaks in the pars plicata of ciliary body

　毛様体皺襞部裂孔による網膜剥離において皺襞部裂孔が小さい場合には，ジアテルミー凝固とインプラントによるバックリングにて良好な手術成績が報告されているが，裂孔が半周を越える広範囲に存在する場合については手術方法は確立されていない。

　症例は両眼の裂孔原性網膜剥離にて当科を受診した15歳のアトピー性皮膚炎患者で，両眼にバックリング手術を施行，網膜裂孔は閉鎖されたが，両眼とも半周を越える広範囲の毛様体搬嚢部裂孔により網膜剥離が再発した。これに対し，まず両眼の強膜輪状締結術を施行，網膜下液の消失した1週間後にバックル上の毛様体扁平部と最周辺部の網膜を全周光凝固した。術後経過は良好で，両眼とも網膜は復位し，その後他院にて両眼のアトピー性白内障の手術を受けるも網膜剥離は現在のところ再発していない。

　毛様体皺襞部裂孔が半周を越える広い範囲にあったり，その疑いがある場合には，眼球の全周にわたり鋸状縁の前後で網脈絡膜瘢痕を作るとともに永続するバックル効果を残す必要があると考えられる。この場合，2段階手術は全周にジアテルミーや冷凍凝固を行うのに比べ，手術侵襲も少なく安全な方法と考えられた。

A 15-year-old boy presented with atopic dermatitis and rhegmatogenous retinal detachment in both eyes. He was initially treated by cryopexy and segmental episcleral buckling. Although the retinal breaks were closed, retinal detachment recurred in both eyes with multiple breaks extending over 180 degrees in the pars plicata of ciliary body. Both eyes were treated by additional scleral buckling over the whole circumference. After reattachment of the retina, laser was applied to the peripheral retina and the pars plana over the whole circumference. He later underwent bilateral cataract surgery. The retinal detachment had been in cured state. We advocate this two-step approach because of its safety, effectiveness and lesser surgical insult.
