内視鏡による毛様体皺襞部裂孔の観察

Endoscopic observation of break in the pars plicata of ciliary body

　近年アトピー性皮膚炎に伴う網膜剥離の原因として，毛様体扁平部あるいは毛様体皺襞部の裂孔が知られている。アトピー性皮膚炎に伴った両眼性の毛様体皺襞部裂孔を原因とする網膜剥離の1例に対して，水晶体切除と強膜バックリングの同時手術を行い，術中内視鏡を用いて裂孔の観察を行った。水晶体の摘出前後に前房穿刺創より内視鏡を挿入することで，裂孔部を含めた毛様体皺襞部を明瞭に観察できた。裂孔は毛様突起のほぼ先端部に形成され，剥離した毛様体無色素上皮は水晶体嚢と接していた。

We performed endoscopic observation of break in the pars plicata in a case of atopic dermatitis with bilateral retinal detachment. An endoscopic probe was inserted through the site of paracentesis before and after lensectomy. The pars plicata was clearly visible through the endoscope. The break was located on the posterior portion of ciliary process. The detached nonpigmented ciliary epithelium was apparently attached to the lens capsule.
