網膜剥離手術後にみられる黄斑部皺襞形成について

Postcoagulative Macular Puckering in Retinal Detachment

I．緒言

　網膜剥離手術の目的は，単に剥離せる網膜を解剖学的に復位させるだけでなく，機能的な回復を目差していることはいうまでもない。とりわけ，中心視力，すなわち黄斑部の機能の回復を重要視するがゆえに，周辺部の視野を時には犠牲にしてまで，黄斑部の網膜の復位と機能回復を計るのである。ところが最近われわれは，網膜剥離を手術によつて一旦完全に復位せしめ，良好な視力をえたにもかかわらず，数週ないし2〜3カ月を経て，徐々にあるいは急速に黄斑部に著明な皺襞形成，周囲血管の蛇行，黄斑部偏位などの変化をきたし，視力が著しく低下した6名の症例を経験した。

We reported six cases of postcoagulative ma-cular puckering with reduced central vision, which appeared from a few weeks up to a few months after coagulation-diathermy or light coagulation- and scleral buckle with silicone-sponge had been successfully carried out for the retinal detachment. Preoperative fundus findings in all these six cases showed high bullous de-tachment extending near to the macula and relatively large tears, the posterior margin of which extended posterior to the equator.

As to the pathogenesis for the macular chan-ges we should like to take the following me-chanism into consideration.
