Cryoretinopexyによる裂孔原性網膜剥離手術の臨床的評価

Cryoretinopexy as a therapeutic method of retinal detachment

　裂孔原性網膜剥離158例，160眼に対し，cryoretinopexy, episcleral buckling，排液を主とする術式を，特に適応を定めずに施行した。その結果を，diathermy凝固，intrascJeral buckling，排液を主とする術式を施行した，187例，191眼と比較しつつ，cryo—retinopexyの臨床的評価を試みた。

　その結果，cryoretinopexy症例においては，91.3％の治癒率を得，以下のような利点，欠点を指摘しえた。

　利点：（1）網膜裂孔周囲を直視下において，確実に凝固できる。（2）合併症としての眼内出血が少ない。

　欠点：（1）初回手術無効例および再発例の原因として，網脈絡膜癒着の弱さと眼内隆起の平坦化を指摘できる。（2）合併症として，exudative retinal detachment, cryoretinopexyによる新裂孔形成を認めた。（3）無効例の原因としてMPPを高頻度に認めた。

　以上より，cryoretinopexyはすぐれた治療法ではあるが，その特徴を十分念頭におきつつ施行する必要がある。

We evaluated the surgical outcome of rhegma-togenous retinal detachment treated by cryoretino-pexy in 160 eyes during the past 2 years. The findings were compared with another series of 191 eyes treated by diathermy during the 32 months ending August 1977. In the series treated by cryo-pexy, drainage of subretinal fluid and episcleral buckling were performed. In the series treated by diathermy, drainage of subretinal fluid and intra-scleral buckling were performed as a routine pro-cedure.

The success rate for the 160 eyes treated by cryo-retinopexy was 91.3%.
