放射線による網膜症と視神経症の4例

Four cases of radiation retinopathy and optic neuropathy

　上顎癌2例，鼻咽頭癌1例，神経膠芽腫1例の放射線照射後に発生した放射線網膜症および視神経症を経験した。これら4例は56Gyから64Gyの放射線照射を受け，治療終了から発症までの期間は平均1か月から36か月（平均16.5か月）であった。網膜症では網膜出血，軟性白斑，硝子体出血などがみられ，血管新生緑内障が1眼にみられた。組織学的検索（症例4）では網膜血管の肥厚，虹彩新生血管，隅角閉塞がみられた。視神経症では視神経乳頭の蒼白，浮腫あるいは乳頭炎の所見を示していた。

We observed retinopathy and optic neuropathy in 4 patients after radiation for malignancies in the paranasal sinus or the brain. The dosis ranged from 56Gy for 14 days to 64Gy for 32 days. The interval between the termination of radiation and onset of fundus lesions ranged from 1 to 36 months, average 16.6 months. The retinopathy appeared as retinal hemorrhage, soft exudates and vitreous hemorrhage. Neovas-cular glaucoma developed in one eye. The optic neuropathy appeared as pallor of optic disc, disc edema or optic papillitis. Histological studies of one eye with retinopathy showed thickening of retinal capillary walls and rubeosis iridis with angle closure.
