病態の異なる眼虚血症候群の2例

Two cases of ocular ischemic syndrome with different manifestations

　眼虚血症候群の2例を経験した。症例1は65歳男性で左眼の突然の視力低下を覚え受診したところ，左眼網膜周辺部の無血管野および新生血管緑内障を認めた。頸動脈造影の結果，内頸動脈の90％にわたる狭窄があり，種々の治療にもかかわらず失明した。症例2は71歳女性で左眼霧視を主訴に受診し，静脈うっ滞網膜症を認めた。眼動脈起始部付近の狭窄と考えられたが，視力は0.3で以後の病態に変化はなかった。眼虚血症状をみた時は全身検査を含めて適切な検査を行い，病態を正確に判断し，新生血管緑内障といった重篤な合併症を生じないように早期治療の必要がある。

A 65-year-old male presented with sudden visual loss in the left eye. He showed capillary nonperfusion in the peripheral retina and neovascular glaucoma. Digital subtraction angiography showed 90% stenosis of the left internal carotid artery. The affected eye became blind despite percutaneous transluminal angioplasty. A 71-year-old female presented with blurred vision in the left eye. Venous stasis retinopathy was present. Magnetic resonance imaging, MRI, showed stenosis of the left ophthalmic artery.She maintained visual acuity of 0.3 in the left eye. These cases illustrate that early diagnosis and treatment may improve prognosis quoad visum in cases of ocular ischemic syndrome.
