ベーチェット病による結膜潰瘍の1例

Conjunctival ulcer in a case of Behçet disease

　内科で1週間前にベーチェット病と診断された40歳の女性が，両眼異物感を主訴として受診した。両眼の球結膜に充血を伴う潰瘍がみられた。前眼部と眼底には炎症所見はなかった。生検で，潰瘍底に好中球とリンパ球などの炎症細胞の浸潤と血管炎があった。抗生物質の点眼は無効であり，副腎皮質ステロイド薬の点眼の併用後に結膜潰瘍が改善した。結膜潰瘍と口腔内アフタが同時期に出現したことと，ステロイド薬の点眼で改善したことから，ベーチェット病による結膜潰瘍と診断した。本邦での報告はないが，結膜潰瘍の原因としてベーチェット病の可能性が考慮されるべきである。

A 40-year-old woman presented with foreign body sensation in both eyes. She had been diagnosed as Behçet diasease due to systemic manifestations one week before. Conjunctival ulcer with hyperemia was present in both eyes. There were no signs of inflammation in the anterior chamber or the fundus. Biopsy of the ulcerated area showed vasculitis and infiltration of lymphocytes and neutrophils. The conjunctival ulcer was diagnosed as ocular manifestation of Behçet disease as it responded favorably to topical corticosteroid and as it developed concurrently with aphthous stomatitis. Behçet disease is a possibility in the etiological diagnosis of Behçet disease.
