水庖性角膜症の全層角膜移植

Penetrating keratoplasty for bullous keratopathy

　46眼の水庖性角膜症に対し全層角膜移植を行った。18眼が透明治癒し，39％の透明治癒率であった。混濁に至った直接の原因は内皮機能不全（21眼）と免疫反応（7眼）であった。若い角膜提供者から得られた移植片では内皮機能不全の発症が有意に少なく，免疫反応が発症した場合，若い角膜提供者からの移植片の方が透明治癒しやすかった。混濁に至った移植片と透明治癒した移植片を比べると，透明治癒した移植片の角膜提供者年齢は混濁に至った移植片の角膜提供者より有意に若かった。年齢ごとに検討した結果，51歳以上の角膜移植では，50歳以下の角膜を移植した場合と比べ，透明治癒率は有意に低かった。

We performed penetrating keratoplasty in 46 eyes during a 9-year-period ending 1989. The series included previous radial keratotomy 31 eyes, cataract surgery 9 and others. Transparent cornea resulted in 18 eyes, 39%. Postsurgical opacity in 28 eyes was due to endothelial dysfunction 21 eyes and immune reaction 7 eyes. Final postsurgical opacity of both types occurred at a significantly higher rate when the donor was aged over 51 years than under 50 years (p ＜0.05).
