It has been established that Kearns-Sayre syndrome (KSS) and chronic progressive external ophthalmoplegia (CPEO) exhibit the same syndrome of mitochondrial disfunction due to large scale deletion of mtDNA. But there are some differences in the clinical symptoms of KSS and CPEO. We evaluated the clinical symptoms of two cases of KSS and four of CPEO from April 1992 to March 1994 retrospectively. One of the two cases of KSS manifested its first symptom at the age of thirty, as in CPEO. Retinochoroidal degeneration was observed in three of the four cases of CPEO. There was some difficulty in distinguishing KSS from CPEO by clinical symptoms. We speculate the difference in mitochondrial dysfunction causes the different symptoms in the two syndromes
