真性小眼球症に対する白内障手術例

Cataract surgery for nanophthalmic eye

　症例は37歳女性で，真性小眼球症に伴うuveal effusionに対し渦静脈減圧術が行われ，治癒した既往がある。4年後，白内障手術を行ったところ，術後uveal effusionが再発したが，比較的短期間に自然消退した。真性小眼球症に対する内眼手術後のuveal effusionは難治性のことが多いとされるが，以前行った渦静脈減圧術が今回のuveal effusionの消退に有効であったと考えられる。uveal effusionの既往のある真性小眼球症であっても渦静脈減圧術を行ってあれば白内障手術は可能であると思われる。

A 37-year-old female had been suffering from bilateral nanophthalmos and excessive hyperopia since childhood. She developed retinal detachment in the left eye 4 years ago. We diagnosed her as uveal effusion and performed surgical decompression of vortex veins and drainage of subretinal and subchoroidal fluid. Retinal detachment subsided 7 months later. She was seen again due to advanced cataract in the left eye. Total retinal detachment was detected immediately after phacoemulsification and aspiration with intraocular lens implantation. Retinal detachment started to spontaneously subside 5 days later and disappeared after 4 months. Previously performed decompression of vortex veins appeared to have led to favorable outcome of uveal effusion during cataract surgery.
