A 71-year-old female presented with acute angle closure glaucoma in both eyes with nanophthalmos. Ultrasound biomicroscopy of the left eye showed shallow anterior chamber, extremely narrow chamber angle and thickening of the pars plana. It appeared that the angle closure glaucoma was induced by anterior displacement of the edematous ciliary body secondary to congested vortex veins. Anatomically large crystalline lens in nanophthalmos seemed to be another contributing factor. Expulsive hemorrhage developed during surgery for cataract and glaucoma in the right eye. This case illustrates the usefulness of ultrasound biomicroscopy in the etiological diagnosis of angle closure glaucoma in nanophthalmos.
