角膜形状解析装置を用いて早期診断できた円錐角膜の7症例

Seven cases of early keratoconus diagnosed by corneal topography

　角膜形状解析装置を用いて診断した早期円錐角膜7例13眼について報告した。診断には形状測定によって得られたマップ表示のほか，I-S valueという定量的評価のパラメータも用いた。マップ表示により下方スティープを示した12眼は，I-S valueにても有意な下方スティープを示し，定性的および定量的な両面から初期円錐角膜であると診断された。また，マップ表示にて直乱視のパターンを示した2眼のうち1眼は，I-S valueから初期円錐角膜と考えたほうがよいと思われた。角膜形状解析装置を用い，定性的および定量的な評価に基づいて検査，診断していくことが円錐角膜の早期発見につながり，その進行予防を含め積極的な対処を可能にするものと考えた。

We could diagnose early keratoconus in 13 eyes of 7 patients using corneal topography. Eyes with early keratoconus showed no abnormal findings in the cornea when examined by biomicroscopy or photokeratoscopy. Corneal topography in 13 eyes showed a pattern of inferior steepening in color-coded map. All the 13 eyes showed significantly positive I-S value indicating significant inferior steepening. Among 2 eyes which showed a figure 8 pattern, only one eye showed significantly positive I-S value and was diagnosed as early keratoconus. Qualitative and quantitative analyses using corneal topography would thus contribute to early detection of keratoconus.
