高齢で初発したベーチェット病の1例

Initial onset of Behcet disease in a senile person

　高齢で初発したぶどう膜炎患者で，ベーチェット病不全型と診断したが，遺伝的疾患抵抗性因子の1つであるhuman leucocyte-associated antigen（HLA）—B44が陽性であった1例を報告した。患者は74歳の女性で，両眼の視力障害を起こした。初診時の矯正視力は右0.3，左0.05で，両眼とも軽度の虹彩炎，硝子体下方の混濁，著明な乳頭浮腫，乳頭周囲網膜の浮腫と線状出血を伴った網脈絡膜炎があった。皮膚紅斑，口内アフタが出現したことからベーチェット病不全型と診断した。検査所見ではHLA-B51が陽性であったが，遺伝的疾患抵抗性因子の1つであるHLA-B44も陽性であった。ベーチェット病の遺伝的疾患抵抗性因子は高齢発症とも関係していると考えられた。

A 74-year-old female presented with bilateral visual disturbance since 4 months before. The initial visual acuity was 0.3 right and 0.05 left. Both eyes showed signs of mild iritis, opacity in the inferior vitreous, marked papilledema and chorioretinitis with retinal edema and linear hemorrhages in the peripapil-lary area. She later manifested erythema and aphthous stomatitis and was diagnosed as incomplete Behcet disease. Laboratory studies showed HLA-B51 and HLA-B44 which is one of disease-resistance genes. Presence of HLA-B44 seemed to have led to manifestation of Behcet disease at an advanced age in this patient.
