マイトマイシンCを用いた翼状片手術の基礎的検討と治療成績

Experimental and clinical studies of pterygium surgery with mitomycin C

　マイトマイシンC （MMC）は，線維芽細胞増殖抑制作用を有しており，翼状片再発防止に有用である。筆者らは，術中にMMCを浸したMedical Quick Absorder （MQA）の切片を強膜に接触させるという新しい手術方法を考案した。有色家兎を用いた基礎実験では，0.02mgMMC含有MQAを用いた場合に，強膜に侵襲を及ぼすことなく，線維芽細胞の増殖が抑制されているのが確認された。さらに同量のMMCを用いた臨床成績では，翼状片手術施行後6か月以上経過観察できた86例99眼を対象とした。1眼（1.0％）に再発を認めたが，術中・術後に合併症を認めず，ほぼ良好な結果を得た。以上より，本法によるMMCの至適量は0.02mgであると考える。

We designed a surgical method for pterygium in which a piece of paper sponge impregnated with mitomycin C is applied to the bared sclera. A piece of paper sponge with 0.02 mg mitomycin C inhibited proliferation of fibroblasts without injury to the sclera in pigmented rabbits. A similar method was used during surgery for pterygium in 99 eyes. After observation for 6 months and longer, no complication developed except recurrence of pterygium in one eye, 1.0%. The finding indicates the optimum concentration of mitomycin C to be 0.02 mg.
