上皮成長因子と硝子体手術による黄斑円孔の治療

Epidermal growth factor as adjunct to vitreous surgery for idiopathic full-thickness macular hole

　3期と4期特発性全層黄斑円孔16眼に対する上皮成長因子（epidermal growth factor：EGF）を用いた硝子体手術の有効性を検討した。硝子体皮質を充分に剥離除去し，高濃度（10μg／200μl）のEGFを使用し，術後厳密な伏臥位を取らせた場合に，14眼（88％）の円孔閉鎖が得られた。術後0.5以上の矯正視力は6眼（38％）に得られた。円孔閉鎖には2種類観察され，これは2種類の黄斑円孔形成過程と関連があるものと推測した。

We evaluated the efficacy of topical application of epidermal growth factor (EGF) during vitreous surgery for 16 eyes with idiopathic full-thickness macular hole. After complete removal of cortical vitreous, EGF solution, 10 μg/200 μl, was applied to the hole. Patients remained in a face-down position for 72 hours. Flattening of macular hole resulted in 14 eyes, 88%. Visual acuity of 20/40 or better was obtained in 6 eyes, 38%. Side effects were absent throughout.
