広範な網膜硝子体出血を伴った結核性汎ぶどう膜炎の1例

A case of tuberculous panuveitis with widespread vitreoretinal hemorrhage

　広範な網膜硝子体出血を伴う結核性汎ぶどう膜炎の1例を経験した。患者は20歳男性で，初診時に虹彩毛様体炎と網膜静脈炎，網膜硝子体出血が両眼にあり，左眼には隅角結節がみられた。ツベルクリン反応は6×5mmの発赤，血清アンジオテンシン変換酵素は正常であった。胸部X線検査で右肺尖部に結節状の陰影を認めたが，経気管支肺生検では診断を確定できなかった。両眼の硝子体出血が増強したため，ステロイド剤と抗結核剤の併用療法を開始した後に，前斜角筋リンパ節生検により結核性リンパ節炎の組織診断を得た。左眼は，薬物治療にて網膜血管炎の改善がみられ，網膜血管閉塞領域への光凝固を施行した。右眼は黄斑部病変と硝子体出血のため，視力は不良であった。

A 20-year-old male presented with impaired vision of one month's duration. Both eyes showed iridocyclitis, retinal phlebitis and vitreoretinal hemorrhage. Chest X-ray showed nodular shadow. Trans-bronchial lung biopsy was negative. PPD skin test showed 6 mm of erythema. Anti-tuberculous therapy with systemic corticosteroid was started under tentative diagnosis of ocular tuberculosis. Later, tuberculosis was diagnosed by scalenus lymph node biopsy. The left eye responded well to the therapy and to laser photocoagulation applied to the avascular area. The right eye failed to respond to treatment.
