松果体腫瘍に対する治療と予後—追跡調査による検討

Radiation Therapy and Surgical Management of Pineal Tumors:Follow up Study of 96 Patients'

　欧米諸国に比較し，わが国においては松果体腫瘍の発生頻度が著しく高いことはよく知られている^{1，4，6，16，19，20）}．したがってその治療法に関しては関心がはなはだ高く，従来も多くの報告がなされてきた^{7，17，18）}．そしてその治療方法として松果体腫（Two cell pattern pinealoma）は放射線によく反応するため照射療法がきわめて有効であることはつとにみとめられ^{2，5，12，13）}，われわれもそれに一致する報告をしてきた^7，8）．今回はさらに自験例をふりかえり，治療法を再検討してみた．

　症例は昭和10年より48年までに経験した松果体腫瘍96例（追跡調査が不能であった1例を除き，いわゆる異所性松果体腫Ectopic pinealomaを含む）で，これらについて行なった治療，特に照射および手術療法を中心にその成績を分析した．

This is a follow-up study of 96 patients with pineal tumors, except for one patient who was lost to follow-up. 60 of these tumors were verified histologically either at operation or at necropsy, 6 were diagnosed by epitheloid cells existing in the cerebrospinal fluid, and 30 were diagnosed by clinical and ventriculographic findings.

In agreement with Russell and Rubinstein, we have classified the histologically verified tumors into 3 groups, as follows; 1. True Pinealoma Group (Pineocytoma, Pineoblastoma), 2. Germinoma-Teratoma Group, 3. Others (Glioma, Cyst).
