強直性脊椎炎にみられた後部ぶどう膜炎

Posterior uveitis in a case of ankylosing spondylitis

　強直性脊椎炎の確実例にみられた後部ぶどう膜炎を経験した。症例は，40歳の男性でHLA-B27陽性を呈し，右眼の霧視・羞明を主訴に当科を受診した。視力は，右0.5，左1.2で，右眼硝子体中に微塵状混濁を認めた。検眼鏡所見では，右眼に黄斑浮腫と網膜静脈の拡張を認めたが，左眼に異常はなかった。螢光眼底造影では，右眼に網膜血管炎の所見とともに，多発性の点状過螢光が数多く認められた。硝子体螢光濃度測定にて血液網膜棚の機能障害が観察され，網膜色素上皮の障害が考えられた。これらの所見はステロイドの大量療法により改善し，右眼視力も1.2へ回復した。

A 40-year-old male presented with blurred vision and photophobia in his right eye. The visual acuity was 0.5 right and 1.2 left. He had been diagnosed as ankylosing spondylitis 20 years before. Fundu-scopy showed hazy vitreous, macular edema and dilated retinal veins in the right eye. Fluorescein angiogra-phy showed diffuse retinal vasculitis with multiple dye leakage spots. Vitreous fluorometry showed disturbed blood-retinal barrier. Systemic corticosteroid induced disappearance of these lesions and improved visual acuity of 1.2.
