高齢で発症したベーチェット病の1例

First ocular attack of Behçet disease in a senile person

　73歳の高齢で眼症状が初発したベーチェット病患者を経験した。眼症状は片眼の虹彩炎で，眼底には変化を見なかった。患者は70歳で口内アフタと結節性紅斑を発症し，後に陰部潰瘍が発生した。眼科初診時にはこれらの全身症状は軽快していた。患眼は老人性白内障の術後で眼内レンズ挿入眼であり，しかも手術に用いられた強角膜創の縫合糸が球結膜から露出していたとの前医の紹介から，虹彩炎の原因として感染が疑われ確定診断に手間取った。ベーチェット病の高齢者の発病は少ないので筆者らの経験を紹介した。

A 70-year-old woman underwent extracapsular cataract extraction and intraocular lens implan-tation in her left eye. She was diagnosed as Behçet disease 6 months later after developing oral aphtha and nodular erythema followed by genital ulcer. Acute iritis in the left eye developed at the age of73 years. Ther was no pathological finding in the fundus. The systemic symptoms had already sub-sided. The iritis was finally diagnosed as manifesta-tion of Behçet disease after ruling out bacterial eye infection because of extrusion of the corneoscleral suture. This case is an unusual instance of Behçet disease with the first ocular attack in a 73-year-old person.
