網膜減張切開・切除術の術後成績

Results of relaxing retinotomies and retinectomies

　網膜減張目的で網膜切聞・切除術を施行した33例，35眼について，術前の状態および手術手技がどの程度予後に影響するかを調べるために，予後に影響しそうな項目について統計学的に検討した。その結果，術前視力0.02以上，増殖性硝子体網膜症で90°未満の範囲の切開は成功率が有意に高かった。そして90°未満の切開で術後低眼圧を来した症例はなかった。また術前視力光覚弁，術中の著明な出血，増殖性硝子体網膜症で下方の切開は有意に予後不良因子であった。本術式を選択する際には注意すべき要因と考えられた。

We reviewed a consecutive series of 35 eyes in 33 cases treated by retinotomy or retinectomy to eval-uate which perioperave factors influenced visual and anatomical success. Factors predisposing to success included preoperative visual acuity of 0.02 or better in general and less than 90 degrees of retinotomy for proliferative vitreoretinopathy. No eye developed phthisis bulbi after retinotomy of less than 90 degrees. Factors predicting failure included preoperaive visual acuity of light percep-tion, massive intraoperative hemorrhage and, for proliferative vitreoretinopathy, inferior location of retinotomy or retinectomy.
