高度近視白内障手術時の術後目標近視度と患者の満足度

Patient's satisfaction regarding postoperative refractive pseudophakic power in former high myopic eyes

　過去4年間に白内障手術を施行し，後房レンズを移植した高度近視患者77名109眼につき，手術成績を検討した。またアンケート調査を実施し，その満足度についても検討した。術後屈折度は−5D前後に設定して手術を施行した。術後網膜剥離の発症は後嚢切開後に1例であった。アンケート結果では，87％で遠くは見やすくなったと答え，近見時は，62％で眼鏡を使用しなくてもよく，87％で手術をしてよかったと答え，満足度は高かった。術後合併症の頻度は低く，高度近視眼にも，術後屈折度を−5D前後に設定された後房レンズ移植は有用であると考えられる。

We evaluated 109 high myopic eyes in 77 patients after cataract extraction and posterior chamber intraocular lens (IOL) implantation. The satisfac-tion of the patients was determined by question-naire. As a regular rule, the postoperative re-fractive power was designed to lie around -5.0 diopters. The incidence of postoperative complica-tions was very low including retinal detachment. 87% of patients expressed their satisfaction regard-ing distant vision. 67% of patients were satisfied with near vision. The refractive power of those complaining near vision ranged from -4.25 diopters to slight hyperopia. 87% of patients were happy with the surgery. The findings indicate that IOL implantation ought to be carried out even in high myopic eyes with cataract and that the post-operative refractive power is to be designed to lie at -5.0 diopters or more.
