網膜剥離を伴う糖尿病性網膜症眼の硝子体手術成績—2．視力不良眼の検討

Pars plana vitrectomy for proliferative diabetic retinopathy with retinal detachment.:2. Factors for post-operative poor visual acuity

　網膜剥離を伴う糖尿病性網膜症126眼の手術成績を，術前測定したEER（electrically evoked response）および汎網膜光凝固の有無との関連で検討した。

　EER不良群に視力改善例が少なく，また剥離網膜の復位しなかったものが多かった。術前施行された汎網膜光凝固のうち，argon laserによる症例群で予後不良例の割合が多かった。

　さらに予後不良例を術中裂孔形成と視神経萎縮様外観を呈した群で検討したところ，術中裂孔形成はEER不良眼に多く，術後視神経障害はargon laserによる汎網膜光凝固を受けた眼に有意に多かった。そしてこれに対する星状神経節ブロック療法は無効であった。術前argon laserによる汎網膜光凝固施行眼で，術後視力を回復した症例は全て黄斑部非剥離眼であった。

　このことから，EER不良眼では術中の網膜裂孔形成に細心の注意を払う必要があること，argon laserによる汎網膜光凝固施行眼では，硝子体手術は網膜剥離が黄斑部に及ぶ前に行われた方がよいことが示唆された。

We evaluated the results of pars plana vitrectomy in 126 eyes with retinal detachment secondary to proliferative diabetic retinopathy during a 3-year period ending 1982. We paid particular attention to the two following factors as correlated with the surgical outcome : electrically evoked response(EER) performed prior to surgery and the past history of scatter photocoagulation with xenon or argon laser before onset of tractional retinal detachment.

Cases with poor EER seldom achieved improved visual acuity. Retinal detachment in such eyes tended to persist after surgery.
