眼窩，眼瞼，球結膜の悪性リンパ腫の治療方針

Therapeutic guidelines for malignant lymphoma of ocular adnexa

　眼窩，眼瞼，球結膜下の悪性リンパ腫の治療について検討した。対象は当科にて免疫組織学的に悪性リンパ腫と診断された19例である。これらに対し外科的切除の後，化学療法，放射線療法あるいは化学療法と放射線療法の併用治療を行った。完全切除が行われた症例は予後が良好であった。Ⅰ期B細胞型悪性リンパ腫14例について検討すると，生命予後は化学療法単独，放射線療法単独よりも併用療法が良好であった。転移の予防については化学療法が有効であった。悪性リンパ腫の治療はⅠ期であっても外科的切除に加えて化学療法と放射線療法を併用することが望ましい。

We reviewed 19 cases of primary malignant lymphoma in the conjunctiva, eyelid or orbit. The diagnosis was confirmed histopathologically. All the cases were treated by surgical excision fol-lowed by chemotherapy and/or radiation. The clin-ical outcome was more favorable in cases with complete excision than others. The present seriesincluded 14 cases of stage 1 B cell lymphoma. The final outcome was better when treated by combined therapy than with chemotherapy or radiation alone. Chemotherapy appeared to be effective to prevent metastasis. Adjuvant chemotherapy is advocated to improve the rate of cure in stage 1 malignant lymphoma treated by regional radiation therapy.
