原発性胆汁性肝硬変症に合併した眼球突出の1例

Exophthalmos in a patient with primary biliary cirrhosis

　自己免疫疾患の一種と考えられる原発性胆汁性肝硬変症（PBC）に合併した眼球突出の症例を経験した。症例は初診時33歳の女性でステロイド反応性の再発性の右眼球突出で近医から紹介された。全身検索でPBCの合併が認められたが甲状腺疾患はなかった。当初眼球突出は比較的軽度で視力も良好であったが，約6年間の経過観察中次第に右眼球突出が著明となり，圧迫性視神経萎縮をきたし，バセドウ病にみられる悪性眼球突出の様相を呈した。眼球突出はバセドウ病と関連が深いが独立した自己免疫疾患であることを示唆する症例と思われる。

A 33-year-old female presented with recurrent exophthalmos in her right eye. The exophthalmos had responded well to systemic corticosteroid. Systemic examinations disclosed the presence of primary biliary cirrhosis. Thyroid disease could beruled out. The exophthalmos gradually progressed during 6 years of follow-up to simulate malignant exophthalmos. Optic atrophy due to compression developed. This case seemed to illustrate than exophthalmos may result from an unspecified autoimmune disorder without thyroid involvement.
