眼瞼の再建における硬口蓋粘膜の応用

Use of hard palate mucosal graft in eyelid reconstruction

　悪性腫瘍摘出により生じた眼瞼の全層欠損において，硬口蓋粘膜による後葉の再建を8例に行った。硬口蓋粘膜は厚く，さらに厚く硬い粘膜下組織を有しており，十分量の移植片を容易に得ることができた。彎曲も適当で，十分な緊張を保ち，術後の収縮も軽度であった。内反や外反はみられず，認むべき角膜障害はみられなかった。移植片採取部は約3週で完全に治癒した。硬口蓋粘膜は採取による犠牲がきわめて少なく，粘膜および瞼板を同時に再建する供給源として最適な組織と思われる。

We performed eyelid reconstruction in 8 patients who had undergone excision of malignant tumor. We used hard palate mucosal graft as substitute for the palpebral conjunctiva. Sufficient graft was obtained with ease, consisting of a thick mucous membrane and a thick hard submucous connectivetissue. The grafts provided proper curve and ten-sion and showed minimal tendency to shrinkage after surgery. No case showed ectropion, entropion or corneal damage. The donor site healed complete-ly in 3 weeks. Hard palate mucosa can be an ideal substitute for simultaneous reconstruction of the tarsus and palpebral conjunctiva.
