　ヘス表を用いて眼窩底骨折の眼球運動障害を分析した。正常のヘス中心座標軸の中心座標点をA点，上30°をB点，下30°をC点とし，BC間の距離を測定した。術前眼球運動障害は，A点に偏位のない症例では上転障害のみの症例と上下転障害のある症例が多くみられた。A点に偏位のある症例では上下転障害のある症例が90％前後を占めた。次に術前期間14日を境に新鮮例と陳旧例に分けた。術前に上転障害のみ，下転障害のみの症例は術前期間と予後に関係はみられなかったが，上下転障害のある症例は，術後BC間が50°未満の眼球運動障害を残した症例は，新鮮例7眼（9％）に対し陳旧例では9眼（38％）あり，早期の手術が必要と考えられた。

We quantitated the ocular motility disturbance following blowout orbital fracture in 177 eyes. The central point of Hess chart was designated as A, a point 30 degrees superior as B, and a point 30 degrees inferior as C. The distance between B and C served as parameter for disturbed ocular motilitygraded into 4 levels.

In the presence of disturbed ocular motility, cases with no deviation at point A showed frequently upward or up-and-down disturbance. More than 90% of cases with deviation at point A showed up -and-down disturbances.

We evaluated the outcome of surgery in cases with up-and-down motility disturbance. The surgi-cal outcome was significantly more favorable when operated within 14 days of injury than later.
