A 56-year-old male presented with idiopathiccorneal endotheliopathy in his left eye. Slit-lampexamination showed severe stromal edema in thelower half of the cornea. Numerous keratic precipi-tates were also present simulating the Khodadoustline arranged linearly along the junction of theedematous and non-edematous cornea. We tappedthe aqueous humor and could demonstrate thepresence of herpes simplex virus (HSV) DNA bypolymerase chain reaction. Topical and systemicacyclovir induced resolution of corneal stromaledema and disappearance of keratic precipitates 2months later. The corneal endothelial lesion recur-red 1 month after discontinuation of acyclovir,when the HSV DNA was repeatedly demonstratedin the aqueus aspirate. These lines of evidencestrongly suggest that this unique endothelial dis-order was caused by HSV infection in the cornealendothelial cells.
