傾斜乳頭症候群に伴う黄斑部障害

Macular degeneration in tilted disc syndrome

　最近5年間に筆者らが経験した傾斜乳頭症候群26例44眼のうち，30眼（68％）に下方強膜ぶどう腫がみられた。この外上縁が黄斑部を横走すると，強膜ぶどう腫の辺縁に沿って網膜色素上皮の変性萎縮を生じる。このような黄斑部障害を21眼（48％）にみた。さらに，2例2眼には黄斑の変性部に，脈絡膜から網膜下に螢光漏出のある網膜浮腫と，1眼の漿液性網膜剥離の合併をみた。傾斜乳頭症候群による黄斑部の色素上皮の変性萎縮は，黄斑部疾患の1つとして取り扱う必要があり，それには眼底の双眼倒像鏡による立体的観察と螢光眼底造影が役立つ。

We reviewed a series of 26 patients, 44 eyes with tilted disc syndrome during the foregoing 5 years. Inferior scleral staphyloma was present in 30 eyes, 68%. Degeneration of the retinal pigment epithe-lium in the macular area was present in 21 eyes. The margin of staphyloma ran through the macula in all these 21 eyes.

Macular edema with dye leakage from the cho-roid to the subretinal space was seen in 2 eyes with macular degeneration. Serous retinal detachment was present in another eye with macular degenera-tion.

The finding seemed to suggest that macular degeneration in tilted disc syndrome formed a distinct clinical entity.
