非定型的な網膜芽細胞腫の1例

A case of atypical retinoblastoma

　コーツ病の疑いで6歳女児が紹介された。右網膜全体に静脈の拡張蛇行，出血，剥離した網膜下に黄白色の濃厚な浸出斑がみられた。網膜下に隆起性病変を思わせる所見があったが，CTおよび超音波検査では腫瘍陰影は検出されなかった。診断的治療の意味でLinac照射を施行し，病変の改善をみたため網膜芽細胞腫を疑い，眼球摘出を勧めたが両親の承諾を得られず，やむなく追加照射を行った。その後CT上，網膜下の腫瘍陰影が増大したため，眼球摘出に至り，網膜芽細胞腫と確定診断した。

A 6-year-old girl was referred to us under the tentative diagnosis of Coats's disease in her right eye. Conjunctival hyperemia of recent onset was the initial complaint. Funduscopy showed dilata-tion of retinal veins and yellowish-white exudatesin the subretinal space. Echography and computed tomography failed to show images suggestive of intraocular tumor. Repeated radiation with Linac resulted in reduction of subretinal exudates sugges-ting the probability of retinoblastoma. Pathological studies of enucleated eyeglobe showed undiffer-entiated retinoblastoma exophytum.
