網膜芽細胞腫におけるIAP（免疫抑制酸性蛋白）測定の意義

Implication of serum immunosuppressive acidic protein in retinoblastoma

　網膜芽細胞腫患児9例の血清免疫抑制酸性蛋白（IAP）を眼球摘出前と摘出後2週に測定した。IAP値と臨床的，病理組織学的所見とを比較することによりIAPの網膜芽細胞腫における意義を検討した。

　緑内障，眼内炎など特記すべき臨床症状を有する症例はIAP高値を示す傾向が認められた。症例がまだ少ないものの，IAP値が網膜芽細胞腫の腫瘍深達度に相関を示す傾向が認められた。

We quantitated the serum level of immunosup-pressive acidic protein (IAP) in 9 patients with retinoblastoma before enucleation of eyeball and 2 weeks after. We correlated the findings with clini-cal and pathological data. Six cases showed normal IAP values of less than 500 μg/ml. Three cases showed definite high IAP values. These 3 caseswere characterized by the presence of additional clinical features as glaucoma or endophthalmitis. They were also unique as they were classified into T3c (choroidal invasion) or T4a (beyond lamina cribrosa) by TNM classification. The 6 cases with normal IAP value were pathologically classified into T3a or T3b. It appeared that serum IAP value in retinoblastoma was well correlated with the grade of tumor penetration.
