巨大な眼瞼脂腺過形成の1例

A case of sebaceous gland hyperplasia of the eyelid

　巨大な眼瞼脂腺過形成の1症例を経験した。症例は81歳男性で，10年前に右上眼瞼の腫瘤に気づき近医にて切開を受け，その後10年間の間に腫瘤は徐々に増大し瞼裂よりはみ出し鼻翼にまで達した。腫瘤の表面は血管に富み赤みを帯びた黄白色で，結節をつくり，ぶどうの房状を呈していた。眼瞼の一部を含めて腫瘤を切除した。病理組織学的には正常の脂腺とほとんど変わらない腺構造を示しており，ズダンⅢ染色では腺細胞の胞体および導管の中に脂肪滴を確認し，脂腺過形成と診断された。その後腫瘍の再発は認められていない。

A 81-year- old man had noticed a small tumor in the right upper eyelid 10 years before. When seen by us, it became a giant tumor with a stalk in the palpebral conjunctivum reaching the wing of the nostril. Its color was reddish to grayish yellow. The whole shape simulated a bunch of grapes. The tumor measured 25×20×50 mm and was excised with a part of the eyelid. Histopathology showed a well-maintained structure containing lipid in the ducts and the cytoplasm resembling normal seba-ceous gland. The condition was diagnosed as hyper-plasia of sebaceous gland which possibly originated from the meibomian gland. No recurrence has been observed.
