細菌感染に続発する難治性角膜障害に対するコラーゲンシールド治療

Collagen shield treatment for serious corneal disorders due to bacterial infection

　細菌性角膜潰瘍に抗生物質治療を行い潰瘍が鎮静化した後にも角膜上皮欠損などの角膜障害が持続する場合がある。コラーゲンシールドは各種の角膜上皮障害に有効であり，薬剤を浸漬して装用させると徐放性のdrug delivery systemとしても有効であると報告されている。今回，細菌感染に続発した遷延性角膜上皮欠損の1例および実質融解の1例に対し抗生物質に浸したコラーゲンシールド治療を行ったところ，上皮欠損および実質融解は速やかに消失した。細菌感染に続発する難治性角膜障害には抗生物質浸漬コラーゲンシールドは有効な治療法であると考えられた。

Persistent corneal epithelial defects are often found in patients with bacterial corneal ulcer treat-ed with antibiotics. We treated two cases of serious corneal problems following bacterial infection; one was persistent epithelial defect and the other was perforation after stromal melting. Successful corneal resurfacing and termination of the stromal melting were achieved by collagen shields contain-ing antibiotics. Use of collagen shields containing antibiotics is one of the methods of choice for the treatment of serious corneal disorders due to bacte-rial infection.
