角膜上皮障害に対するコラーゲンシールドを用いた治療

Treatment of corneal epithelial disorders with collagen shield

　ブタ強膜由来コラーゲン角膜シールドを種々の原因による角膜上皮障害の症例42例に試み，その臨床的有効性について検討した。

　コラーゲン角膜シールドは，びまん性表層角膜炎の4症例に対しては，3例75％に著効を示し，角膜びらん26例に対しては，21例81％に著効あるいは有効，遷延性角膜上皮欠損7例に対しては，5例71％にやや有効以上の判定であった。上皮下浮腫の2症例には，無効であった。

　コラーゲンシールドの装着により角膜表面に対するbandage効果で痛みや異物感の軽減が得られるとともに，溶解した分解物が潤滑剤として作用し，角膜上皮欠損の修復と再被覆を促すと考えられ，角膜上皮障害に対する一つの有効な治療法と評価される。

We treated 42 eyes with corneal epithelial dis-orders by sustained wear of collagen shield with additional instilltion of physiologic saline of aque-ous antibiotics solution. The series included diffuse superficial keratitis 4 eyes, corneal erosion 26 eyes, persistent corneal epithelial defect 7 eyes, bullouskeratopathy 2 eyes and other 3 eyes. The treatment induced improvement in the clinical findings within 2 weeks in 31 eyes (74%). Bullous keratopathy, or subepithelial edema, failed to respond. The obser-ved beneficial responses to collagen shield seemed to be due to bandage and lubrication effects to the corneal surface.
