涙嚢原発の粘液性類表皮癌の1例

A case of primary mucoepidermoid carcinoma of the lacrimal sac

　25歳の男性に，十数年の流涙の後に発症した涙嚢腫瘍を経験した。腫瘍は涙嚢部から鼻涙管まで続いており，組織検査では乳頭状に増殖した上皮性の腫瘍細胞層の間に，粘液産生性の腫瘍細胞が散在し，一部では多量の粘液を産生している所見が認められ，粘液性類表皮癌（mucoepider-moid carcinoma）の診断であった。腫瘍切除術に続いて，上顎部分切除術および眼窩内容除去術が施行され，術後4年を経過し，再発をみていない。術前の検査では，流涙があるにもかかわらず涙道通水試験の通過は良好で，ときどき血液の逆流を見たことが涙嚢腫瘍として特徴的であった。

A 25-year-old male presented with unilateral tumorous mass nasal to the inner canthus. Epiphora had been presented for the past 10 years. The la-crimal pathway allowed free passage of water under pressure with occasional regurgitation of blood. Surgery showed the tumor to be extending from the lacrimal sac along the nasolacrimal duct.

Histopathologically, the tumor contained papil-lomatous growth of epidermoid cells and mucinous deposit surrounded by mucin-producing tumor cells. Mucin was stained positive by PAS, alcian blue and mucicarmine. There has been no recur-rence during 4 years after orbital exenteration.
