後極部小円孔に起因する網膜剥離に対する Cyanoacrylate Retinopexy

Retinopexy with cyanoacrylate for retinal detachment with a small hole in the posterior fundus

　後極部小円孔に起因する網膜剥離に対する手術方法は強膜バックリングよりも三宅の硝子体内ガス注人法とGonvers　＆　Machemerの硝子体切除。internal drainageとガスタンポナーデによる方法が一般的となってきた。しかしながら、時にいずれの方法でも復位が得られない場合がある。黄斑円孔2例，後部ぶどう腫内小円孔2例，後極部小円孔1例および朝顔症候群に合併した乳頭辺縁円孔の計6例に対し，cyanoacrylate　retinopexyを行った。朝顔症候群以外の5例はすべて増殖性硝子体網膜症を伴っており，うち4例は複数の手術後の円剥離例である。全例に復位が得られ，黄斑円孔例においても術後視力は満足すべきものと考えられた。

We performed cyanoacrylate retinopexy in 6 eyes of retinal detachment with a small hole in the posterior fundus. The series included 2 eyes with macular hole, 2 eyes with a hole within posterior staphyloma, 1 eye with a hole in the posterior fundus and 1 eye with peripapillary hole associated with morning glory syndrome. Multiple surgeries had been performed in 4 eyes. Proliferative vitreo-retinopathy was present in 4 eyes. The retina became reattached with improved visual acuity in all the eyes.
