毛様網膜動脈閉塞を伴う網膜中心静脈閉塞症の1例

A case of cilio-retinal arteriolar occlusion after central retinal vein occlusion

緒　言

　最近，著者らは網膜中心静脈閉塞症の所見を伴つた毛様網膜動脈閉塞症の症例について，発症初期より長期にわたる経過を観察する機会を得た。本症例と類似の報告は，著者らの調査し得た範囲内では欧米で13例^1〜2），本邦で2例^3）を数えるのみであり，しかも発症後極く初期の検眼鏡所見および螢光眼底所見をとらえたものは少ない。今回の症例は，自覚症発生から約3時間後に初診しており，その特異な螢光眼底所見は他に類例をみない。

　本稿ではこの症例の経過を詳細に報告し，更に網膜と脈絡膜の循環，および相互の関係を明らかにすることによつて，網膜中心静脈閉塞と毛様網膜動脈の循環障害との関係を論じ，本症候発生の本態解明を試みた。

A female patient, 24 years of age, developed central retinal vein occlusion accompanied by signs of retinal ischemia in the area irrigated by a cilioretinal artery. We could perform fluorescein fundus angiography within three hours after onset of the disease. The central retinal vein occlusion was transitory and resulted in benign and self-limited clinical course. The associated occlusion of the cilio-retinal artery was probably the consequence of decreased arteriovenous pressure gradient be-tween the cilioretinal artery and the central retinal vein while the latter was in occluded state because of phlebitis.
