Abstract Purpose:To evaluate the long-term postoperative outcomes of patients with neovascular glaucoma complicated by proliferative diabetic retinopathy who underwent Baerveldt glaucoma implant(BGI, pars plana model)surgery.

Subjects and Method:This study included 31 eyes of 27 patients(22 men, 5 women)who were followed for more than 5 years after BGI surgery(range, 102-350 months)performed between December 2014 and September 2017. The BGI was inserted after or during the vitrectomy. The mean observation period was 73±11 months. The pre- and postoperative intraocular pressure(IOP), medication scores, logMAR visual acuity, and complications were retrospectively evaluated.

Results:Due to unfavorable IOP control, 13 eyes(42%)required additional glaucoma surgery, while the other 18 eyes(58%)did not require additional surgery;their pre-and 5-year postoperative IOP, at 37.9±14.4 and 14.4±4.1 mmHg, respectively, were significantly reduced. Their pre- and 5-year postoperative medication scores, at 4.9±1.2 and 2.2±1.6, respectively, were significantly reduced. The pre- and 5-year postoperative logMAR visual acuities, at 1.38±0.9 and 0.85±0.6, respectively, showing an improving tendency. Postoperative complications included vitreous hemorrhage in 11 eyes(35%), choroidal detachment in 7 eyes(23%), tube-related complications in 3 eyes(10%), endophthalmitis in 2 eyes(6%), corneal epithelial damage in 2 eyes(6%), and hyphema in 1 eye(3%).

Conclusion:Five years after the BGI surgery, in approximately 60% of cases, IOP was controlled favorably and visual acuity was maintained without reoperation.
