Abstract Purpose:To investigate the efficacy of vitrectomy without internal limiting membrane for myopic macular schisis with good visual acuity.

Materials and methods:Case 1 was of a 64-year-old woman. She presented with metamorphopsia in the right eye. The visual acuity of the right eye was 0logMAR. Optical coherence tomography(OCT)showed maculoschisis with foveal detachment and outer layer macular hole. The central retinal thickness(CRT)was 468 μm. Case 2 was of a 47-year-old man. He presented with metamorphopsia in the right eye. The visual acuity of the right eye was 0logMAR. OCT showed maculoschisis with foveal detachment and outer layer macular hole. The CRT was 444 μm.

Vitrectomy was performed. The posterior vitreous cortex was completely removed up to the vascular arcade.

Result:In case 1, maculoschisis and foveal detachment almost disappeared on the 10th postoperative day. Four months after the operation, mild recurrence of retinoschisis was observed, but visual acuity was maintained at 0logMAR, and CRT improved to 141 μm. In case 2, maculoschisis almost disappeared 2 months postoperatively, and foveal detachment almost disappeared 3.5 months postoperatively. No recurrence was observed until 25 months postoperatively. The visual acuity was maintained at 0logMAR, and CRT improved to 141 μm. There were no intraoperative or postoperative complications in both cases.

Conclusions:Vitrectomy without peeling internal limiting membrane for myopic macular schisis may be a useful surgical procedure with few complications.
