Abstract Purpose:Paracentral acute middle maculopathy(PAMM)is a disease characterized by retinal whitening in the parafovea and sudden onset of visual field loss. We present a patient with PAMM with suspected optic neuritis.

Case:A 74-year-old man visited an ophthalmologist for blurred vision and visual field loss in the left eye, and suspected optic neuritis. The visual acuity of the right and left eyes was 1.5 and 0.9, respectively;the critical flicker frequency(CFF)was 40 Hz and 28 Hz, respectively;and relative afferent pupillary defect(RAPD)was present in the left eye. The RAPD amplitude by RAPDx® was 1.33(log units). Soft exudate temporal to the optic disc and retinal whitening around the parafoveal were found in the left eye. Multiple hyper-reflective lesions were detected in the inner nuclear layer through optical coherence tomography(OCT). Two days later, the visual acuity of the left eye was 0.4, and central and paracentral scotoma were detected by Goldmann perimetry. En face OCT in the left eye clearly showed hyper-reflective lesions in the superficial and deep layers. Accordingly, we diagnosed the PAMM in the left eye. Hyper-reflective lesions in en face OCT disappeared over time, and visual acuity of 0.9, RAPD amplitude of 0.03, CFF of 39 Hz after 6 weeks, and visual function was observed.

Conclusion:PAMM may present with findings similar to those of optic neuritis, including positive RAPD and decreased CFF. Therefore, RAPDx® and en face OCT are useful for differential diagnosis and follow-up in PAMM.
