Abstract Purpose:To report two cases of cytomegalovirus(CMV)retinitis following bendamustine administration.

Case 1:A 68-year-old male patient received bendamustine and rituximab for follicular lymphoma. Nine months after starting the treatment, dilated fundus examination revealed a yellowish-white bilateral lesion with retinal hemorrhage. The aqueous humor was positive for CMV-DNA. Based on these findings, CMV retinitis was diagnosed. The patient received intravitreal and intravenous ganciclovir injections. Despite continuous oral valganciclovir therapy, CMV retinitis recurred three times.

Case 2:An 83-year-old male patient received bendamustine and rituximab for mucosa-associated lymphoid tissue lymphoma. He experienced repeated myelosuppression caused by the anticancer agents. After his immune status recovered, hypopyon and vitreous opacity appeared in the left eye. The aqueous humor was positive for CMV-DNA. Based on these findings, CMV retinitis was diagnosed. He received intravitreal and intravenous ganciclovir injections. Thereafter the inflammation improved, and he continued to receive oral valganciclovir therapy.

Conclusion:CMV retinitis may occur due to CD4 positive T cell suppression following bendamustine administration. During the period of immune recovery, CMV retinitis with severe inflammation may occur.
