VERION®によるトーリックIOL挿入術においてPurkinje像を一致させる必要性の検討

Examination of the need to match Purkinje images in toric IOL insertion with VERION®

要約　目的：VERION^®にてトーリック眼内レンズ（IOL）の軸合わせを行う際に，Purkinje像を一致させる必要があるか検討した。

対象と方法：対象はトーリックIOLを挿入した24眼。挿入したirrigation and aspiration（I/A）でPurkinje像を一致させた。その状態のまま，I/A先端でIOLの軸合わせを行い，VERION^®にて撮影した（A群）。I/A抜去後，自由眼位の状態で再度撮影した（B群）。A群，B群の画像から，VERION投影線と軸マークの角度を測定した。両者の角度差の分布やウィルコクソンの符号付順位和検定を行い検討した。

結果：A群とB群の角度差は0〜1.7°（平均0.77±0.54°）であった。1°未満が15/24（62.5％）であった。A群とB群に有意差はなかった（p＝0.121）。

結論：VERION^®によるトーリックIOLの軸合わせは，Purkinje像を一致させなくてもよい可能性がある。

Abstract　Purpose：To report the necessity of matching the Purkinje images when aligning the toric intraocular lens（IOL）with VERION^®.

Subject and method：Twenty-four eyes with toric IOLs inserted are taken as the subjects. The Purkinje image was matched by the inserted I/A. In that state, the IOL was aligned at the tip of the I/A and photographed using VERION^®（Group A）. After removing the I/A, the image was taken again in the free eye position（Group B）. The angle between the VERION projection line and the axis mark was measured from the images of Groups A and B. The distribution of the angle difference between the two was examined and the Wilcoxon signed rank sum test was performed.

Results：The angular difference between Groups A and B ranged from 0°to 1.7°（mean 0.77±0.54°）. Of the subjects, 15/24（62.5％）had an angular difference of less than 1°. There was no significant difference between Groups A and B.（p＝0.121）

Conclusion：VERION^® alignment of toric IOL may not require matching Purkinje images.
