トーリック眼内レンズ挿入術における眼底カメラを用いた簡便なマーキング方法

A method of marking the cornea using a fundus camera for implantation of toric intraocular lens

要約　目的：眼底カメラを用いてトーリック眼内レンズ挿入術における基準点マーキングを行い，その有用性を検討した。対象と方法：28眼に対して，任意の位置に基準点マーキング後，眼底カメラで前眼部を撮影した。眼底カメラの観察光は小さな白色輪状に角膜中央に写る。前眼部写真に写った基準点と観察光から角度を測定した。同じ基準点を，角膜形状測定装置を用い角度を測定し，両者を比較した。結果：両者の角度が一致したのが11眼，すべて3°差以内であった。Mann-Whitney U検定で両者に有意差はなかった。結論：眼底カメラを利用すれば，角膜形状測定装置がない施設でもほぼ同等の正確さで基準点マーキングを行うことが可能である。

Abstract.　Purpose：To describe a method of corneal marking using a fundus camera for implantation of toric intraocular lens（IOL）. Cases and Method：After arbitrary marking of the cornea,28 eyes were photographed with a fundus camera set at the anterior segment mode. The observation light of the fundus camera appears at the center of the cornea as a small ring. Measurement was made between the marking and the ring. The same measurement was made between the marking and findings by corneal topography. Results：Eleven eyes gave the same values by the two methods. Values in the other 17 eyes differed by 3 degrees or less. Values in the 11 and 17 eyes showed no significant difference by Mann-Whitney U test. Conclusion：Use of fundus camera for reference marking for implantation of toric IOL was as accurate as corneal topographer.
