要約　目的：トーリック眼内レンズの推奨モデルと，推奨モデルより1段階乱視矯正効果の強いモデルを挿入した群の自覚乱視を比較した。対象と方法：推奨モデルSN6AT4の20例（以下，推奨T4群），SN6AT4推奨を変更しSN6AT5の12例（以下，強矯正T5群）において，術後3か月の自覚乱視を比較した。結果：術後自覚乱視が1.0D以下となる割合は推奨T4群13/20（65％），強矯正T5群12/12（100％）であった。有意に強矯正T5群の割合が高かった（p＝0.02303）。結論：推奨モデルより1段階乱視矯正効果の強いモデルへ変更したほうがよい。

Abstract.　Purpose：To report the state of refraction after cataract surgery of toric intraocular lens（IOL）of recommended and one-step higher power for correction of astigmatism. Cases and Method：This study was made on 32 eyes that received cataract surgery with insertion of AcrySof^®IOL. Twenty eyes were inserted with SN6AT4 as recommended. Twelve eyes were inserted with SN6AT5 that is stronger than SN6AT4 by one step. State of refraction was examined 3 months after surgery. Results：Postoperative astigmatism was less than 1.0 diopter in 13 eyes（65％）inserted with SN6AT4 and in 12 eyes（100％）inserted with SN6AT5. The difference was significant（p＝0.02303）. Conclusion：Toric IOL of higher power by one step is recommended for correction of astigmatism.
