Abstract Purpose:To report a case of infected foci in the deep parenchyma of the cornea detached by blunt trauma.

Cases and findings:A 77-year-old man was hit by a pair of pliers flew at his work and bruised his right eye. He visited to the local doctor for 5 days and was referred to the Kagoshima University hospital. Best corrected visual acuity(BCVA)at the first visit was 0.05, and separation of the corneal stroma was observed. Although there was no perforation, an infected foci was found in the detached deep corneal stroma, and inflammation in the anterior chamber with hypopyon was also observed. There was no apparent epithelial defect. Antibiotics cefmenoxime and levofloxacin eye drops were instilled and the separated corneal deep parenchyma was washed several times, but no improvement was gained. After 30 days, curettage of separated corneal deep parenchyma was done followed by through washing. After that, Gram-positive cocci were confirmed by microscopic examination. The infection gradually subsided and BCVA improved to 0.6 after 6 months.

Conclusion:We experienced a rare infectious keratitis in the separated corneal deep parenchyma due to trauma. Curettage plus antibiotics treatment was effective.
