Abstract Purpose:To report a case of a lens fragment that was retained in the anterior chamber for 40 years after surgery.

Case:A 79-year-old man had bilateral artificial aphakia after cataract surgery performed 40 years ago. However, his left vision gradually deteriorated due to bullous keratopathy. Four months before presentation, he noticed visual impairment with anterior uveitis in his left eye. Thereafter, he was referred to us because of the sudden appearance of a mass lesion on the surface of the iris, 1 week before referral.

Findings and Clinical Course:His best corrected visual acuity was 0.06 in the left eye. The left eye showed a mass-like lesion on the lower iris, and advanced bullous keratopathy. He was diagnosed with an iris cyst, which was then surgically removed. There was no continuity between the lesion and the iris, and the removed foreign body was diagnosed as lens tissue upon subsequent histopathological examination. After surgery, the bullous keratopathy did not improve, but there was no recurrence of uveitis.

Conclusion:An intra-cameral lens fragment retained after cataract surgery induced chronic anterior chamber inflammation. Although it looked similarly to an iris cyst, pathological examination proved it to have a different origin.
