頸部鈍的外傷後に咽頭裂創を認めた1例

Pharyngeal Perforation after Blunt Neck Trauma

I．はじめに

　外傷による咽頭裂創は様々な原因で起こるが，頸部鈍的外傷によるものは極めて稀であり，われわれが検索した限りでは本邦ではいまだに報告されていない。咽頭裂創は皮下気腫，縦隔気腫，さらには縦隔膿瘍などを形成する可能性があり，適切な治療が行われなければ，重篤な合併症を引き起こす。

　今回われわれは，交通事故による頸部鈍的外傷により咽頭裂創を生じ，頸部および縦隔に気腫を認め，緊急に裂創閉鎖，頸部ドレナージを行い，軽快・退院し得た1例を経験した。本症例の検討とともに若干の文献的考察を加えて報告する。

Pharyngeal perforation from blunt neck trauma is not common complication. A 30-year-old woman developed neck and left anterior chest pain after motor vehicle accident. Plain computed tomography scan showed mediastinal emphysema and subcutaneous emphysema in the neck. In emergency operation,there was a pharyngeal perforation of 4cm in length with 1cm in maximal width. Perforated wound was surgically closed and a drainage suction tube was placed. Systemic antibiotics were administrated for total 16 days after operation. The patient was discharged on day 21.
