経口抗癌薬TS-1投与後に発症した高度涙小管閉塞症の治療成績

Treatment of severe canalicular blockage secondary to peroral TS-1,an antineoplastic agent

要約　目的：抗癌薬であるTS-1の服用後に発症した涙小管閉塞に対する外科的治療の成績の報告。対象と方法：過去53か月間に，TS-1の服用後に発症した涙小管閉塞8例13側を対象とした。男性7例，女性1例で，原疾患は6例が胃癌，2例が大腸癌であった。涙小管閉塞はすべて高度であり，その程度により，4側にシリコーンチューブ留置，9側に結膜涙囊または結膜鼻腔吻合術を行った。結果：全例で涙道が開通し，自覚的・他覚的に症状が改善した。結論：抗癌薬TS-1の内服で涙小管が閉塞することがある。これに対し，シリコーンチューブ留置，結膜涙囊吻合術または結膜鼻腔吻合術が有効である。

Abstract.　Purpose：To report the outcome of treatment for obstruction of laclimal canalicula secondary to peroral TS-1,an antineoplastic agent. Cases and Method：This retrospective study was made on 13 eyes of 8 cases who developed canalicular obstruction following treatment with TS-1 during the past 53 months. The series comprised 7 males and 1 females. Six patients had gastric cancer and 2 had cancer of the colon. Depending on the severity of obstruction,4 eyes were treated by insertion of silicone tube and 9 eyes by conjunctivodacryocystorhinostomy. Results：Lacrimal passage was reestablished in all the cases. All the cases showed subjective and objective improvements. Conclusion：Canalicular obstruction may result from systemic treatment with TS-1. It can be treated either by silicone intubation or conjunctivodacryocystorhinostomy depending on the severity.
