抗癌剤TS-1(R)内服による角膜障害の1例

A case of corneal lesion following treatment by peroras TS-1(R)

要約　目的：抗癌剤TS-1^(R)を服用中に発症した角膜障害の症例の報告。症例：76歳男性が結腸癌の手術後にTS-1^(R)の服用を開始した。TS-1^(R)にはテガフール，ギメラシル，オテラシルカリウムを含む代謝拮抗薬で抗腫瘍効果があるとされている。1日量100mgを2週経口投与し，2週休薬したのち再び投与する方法をとった。内服開始から11か月後に視力低下を自覚し，その1か月後に受診した。所見：矯正視力は右1.0，左0.8で，右眼には異常がなく，左眼に涙液分泌低下と角膜上皮の凹凸があった。TS-1^(R)の服用を中止し，7か月後に軽快した。左眼角膜の病変に接する輪部には，生検で結膜上皮の角化像があった。結論：本症例はTS-1^(R)に特徴的な角膜障害であると解釈され，これに含まれる薬剤と5-フルオロウラシルが関与している可能性がある。

Abstract.　Purpose：To report a case of corneal lesion following treatment by peroral TS-1^(R). Case：A 76-year-old male had been receiving TS-1^(R) for 11 months after surgery for colorectal cancer. TS-1^(R) contains tegafur, gimestat and potassium oxonate, and is claimed to have anticancer effect. The patient had been given a daily dosis of 100mg of TS-1^(R) for 2 weeks to be repeated after an interval of 2 weeks. He noted blurring of vision 11 months after start of treatment and was seen by us 1 month later. Findings：His corrected visual acuity was 1.0 right and 0.8 left. The right eye showed normal findings. The left eye showed decreased lacrimation and uneven corneal surface. By biopsy, keratinization of epithelium was seen in the limbal conjunctiva. The keratopathy was cured 7 months after discontinuation of TS-1^(R). Conclusion：This case illustrates that corneal disorders may develop following prolonged systemic administration of TS-1^(R).
