胆囊癌に伴う癌関連網膜症の1例

Cancer-associated retinopathy in a patient of gallbladder carcinoma

　80歳女性が2か月前からの急激に進行する両眼の視力低下で受診した。高血圧があり，5年前に無症候性胆囊結石と慢性膵臓炎が指摘されている。視力は右手動弁，左0.15であった。両眼とも，網膜が粗そうで血管が狭細化し，血管アーケード付近に色素沈着があった。蛍光眼底造影で網膜血管の透過性亢進があった。両眼に輪状暗点があり，右眼には中心暗点があった。網膜電図は消失型であった。全身検査で胆囊癌が発見された。総肝管，総胆管，門脈，肝動脈などへの浸潤があり，手術の適応なしと判断された。患者血清のWestern blotによる免疫学的検討で，27 kDaの網膜抗原が同定され，癌関連網膜症（cancer-associated retinopathy：CAR）の診断が確定した。網膜症に対しては治療を行わず，初診から約4か月後に逝去した。

　A 80-year-old woman presented with progressive blurring of vision in both eyes since 2months before. She had been diagnosed with asymptomatic gallbladder stone and chronic pancreatitis 5 years before. Her visual acuity was hand motion right and 0.15 left. Funduscopy showed mottled retina,narrowing of vessels,and pigment deposits along the vascular arcades in both eyes. Both eyes showed ring scotoma. Central scotoma was also present in the right eye. Electroretinogram was extinguished. Systemic examinations showed gallbladder carcinoma with metastasis. Western blotting analysis showed 27 kDa antigen to the retina,leading to the diagnosis of cancer-associated retinopathy. To the best of our knowledge,this is the first reported case of cancer-associated retinopathy secondary to gallbladder carcinoma.
