Abstract Background:Paracentral acute middle maculopathy(PAMM)is characterized by sudden onset of impaired vision and paracentral scotoma. Optical coherence tomography(OCT)shows a band of hyper-reflectivity in the inner nuclear layer. It is considered to be due to ischemia in the deep capillary plexus in the retina.

Purpose:To report five cases of PAMM seen during the past 11 months.

Cases:The series comprised 3 males and 2 females. They were aged 39, 52, 69, 78 and 81 years respectively. All were unilaterally affected. All the cases noted impaired vision in the central or paracentral area as chief complaint. Before been seen by us, 3 cases had been diagnosed with branch retinal artery occlusion, and one with central retinal artery occlusion.

Findings and Clinical Course:Visual acuity in the affected eye ranged from 0.8 to 1.0 when initially seen by us. Three eyes showed edematous retinal opacity near the fovea. OCT showed hyper-reflective band in the middle retinal layer in all the 5 eyes. Fluorescein angiography showed no abnormal findings corresponding to the lesion detected by OCT. All the 5 eyes showed improved funduscopic findings after 3 to 6 months. OCT showed attenuation of middle retinal layer. One cases proved to have had antiphospholid syndrome.

Conclusion:All the present 5 cases of PAMM showed acutely impaired vision or visual field. OCT showed hyper-reflective band in the middle retinal layer in all the affected eyes.
